Once again, Congress looks at a "doc fix" — a way to postpone scheduled cuts in payments to physicians. What to do? They want to curtail Medicare spending without alienating doctors (some have dropped out of Medicare and others threaten to). As the New York Times reports , for 13 years, temporary patches have deferred the payment cuts, potentially making them much worse, if they ever actually occur.

Hospitals are mining their patients' records in search of customers , according to Kaiser Health News , which estimates 20 percent of hospitals, including large academic medical centers and large for-profit chains, are marketing  certain services — mostly the profitable ones — aggressively  to their own patients. Hospitals say they're promoting important things like cancer screenings and cholesterol tests, but they often use the data to selectively target patients with private health insurance, which typically pay higher rates than government coverage.

Why do cardiologists often pass up safe, low-tech treatments for chest pain ? Good question. The Washington Post helps flesh out the obvious answer: cardiologists put in more stents to treat clogged arteries, because they get paid more for it, even though the research shows that many patients would be served as well or better by “optimal medical therapy” consisting of aspirin, beta blockers and statin drugs, along with help losing weight, quitting smoking and keeping blood pressure under control. The Affordable Care Act has incentives to change doctors' behavior, but policy experts debate how potent those incentives will be in getting the desired result -- improved care at a lower cost.
