Wednesday's story on health insurance rates was accurate, but missed the major reason why health costs are rising. Insurance companies play a much smaller role in the increases than your coverage portrayed.

There are four components to premium rates developed and charged: claims cost, administration, taxes and profit. All carriers pay the same taxes and hope to achieve a profit on premium of about 2 percent. The two biggest components are the claims and the cost of doing business. All insurers have been tightening up on their administration expenses over the last decade. The new act set standards that many of the carriers were already at or near.

Medical claims are by far the major cost driver. When claims rise, rates rise. According to the Centers for Disease Control and Prevention, obesity levels have almost tripled in the last 20 years. Almost 26 million people now have diabetes, with an estimated medical cost of $116 billion. Americans are not nearly as healthy as they were a generation ago, and that is reflected in claims.

Americans also have access to the greatest technologies when it comes to medical care. For example, plaster casts have been mostly replaced by expensive, strap-on devices that are lighter and certainly more convenient. MRIs are expensive but excellent technology. All these advancements we love do come with a cost. Look no further than the cost of name-brand, patent-protected drugs versus generic equivalents. I have also seen estimates that up to 30 percent of diagnostic tests ordered are a result of physicians practicing defensive medicine. As malpractice rates rise, so do the fees providers charge.

In 2014, when all coverage is guaranteed-issue, this too comes at a cost. If you knew your roof needed total replacement in two years and would cost about $12,000, you would start saving now. This scenario is no different. The insurers are being told to accept all risk, but without knowing the actual cost. The recent rate increases studied may reflect some sort of savings preparation for the unknown.
